Care & Living Transitions

When a parent’s care needs or living arrangements change, the family can suddenly be coordinating appointments, paperwork, a home, belongings, providers, and difficult decisions at once. The right next steps depend on the person, the family, and what is changing.

Family Transition Office helps families in Austin organize the practical work around a care or living change. We can help map decisions, gather and organize information, coordinate authorized home and vendor logistics, keep family members informed, and track what happens next. We work alongside care teams and other professionals; each stays responsible for their own professional advice and services.

Start with a plan

The $499 Assessment and Guide + Planning Session includes the complete digital guide, one private 75-minute advisory and planning session, and written priorities, decisions, and next steps for your family. It is guidance only. It does not include in-person visits, inventory, estate-sale coordination, vendor coordination, or implementation.

For hands-on coordination

Managed Coordination starts at $4,999 for 30 days. It covers one home, up to three workstreams, and up to three vendors, with site visits as needed within the scope agreed before work begins. Evening and weekend needs can be discussed case by case, and response timing is agreed for each engagement.

What families can expect

At kickoff, we agree on the priorities, who is involved, the scope, and how updates will work. The working checklist records each task, its owner, target date, status, next step, and how the completed item or document was shared. Typical items may include:

• decisions and information the family wants to gather
• care, home, and move-related logistics within the agreed scope
• authorized access and vendor scheduling
• family updates and decisions to confirm
• documents or handoffs to complete before the engagement ends

Every family’s plan is different. We confirm what is included together before work begins. Get the $499 Assessment and Guide + Planning Session.
